The Case Coordinator will focus on supporting the Team Leaders and maintaining strong relationships with participants, their families, and key stakeholders to ensure best practice services align with individual goals and plans.
This role also includes management and development of staff, administrative duties including service agreements, medication system oversight, critical incident reporting and formal supervision.Salary: SCHADS Award Level 4 - Pay Point 1 or *****/hourMain Duties & Performance GoalsOversee and implement Individual plans with a person-centred approachAssist in the intake and induction of new participantsManage and monitor staff,



including behaviour plans and incident reportingBuild relationships with stakeholders for positive participant outcomesAttend relevant NDIS and specialist appointmentsReview participant profiles and introduce new activitiesFacilitate communication and advocate for participants' needsSupervise and manage Team LeadersProvide training and mentoring to Team LeadersMonitor and document the success of IPP goal implementationReport issues and risks to the General ManagerReview medication administration and ensure accurate recordingOversee medication charts and manage changes in CTARSEnsure workplace health and safety, report incidents, and adhere to safe work practicesMaintain documentation, reporting, and safeguarding requirementsProtect the privacy and confidentiality of all participants, staff, and organisationInclude working a rotating roster for the "On Call" serviceMaintain appropriate professional boundaries at all timesQualifications & ExperienceAt least Certificate IV in Individual Support or equivalent requiredNDIS Worker Screening and NDIS Worker Orientation ModuleCurrent First aid & CPRCurrent NDIS Worker Screening CheckCurrent national police check (or willing to obtain)Valid NT Driver licenceEvidence of right to work in AustraliaExperience with any of the following will be highly regarded - case coordination and the ability to develop, implement, monitor and evaluate programs for people with disabilities.Experience in working with Indigenous Australians and people from culturally and linguistically diverse backgrounds.Well-developed written and oral communication skills, particularly an ability to communicate effectively with a wide range of individuals and organizations.Apply directly through this link: Type: Full-timePay: From $***** per hourLicence/Certification:NDIS Worker Screening Check (Required)First Aid & CPR Certificate (Required)NT Driver's Licence (Required)Ochre Card (Required)NT National Police Certificate (Required)Certificate IV in Individual Support or equivalent?
