As our Diabetes Educator, you'll provide personalised education, coaching, and ongoing support to people living with diabetes through one-on-one consultations and group education sessions.You'll help patients understand insulin therapy,



blood glucose monitoring, nutrition, and lifestyle changes while working closely with our GPs and allied health team to deliver coordinated, patient-centred care that improves long-term health outcomes.Key ResponsibilitiesDeliver individual and group diabetes education and self-management supportSupport patients starting or adjusting insulin and other diabetes therapiesWork with GPs and practice nurses on shared care planning for diabetic patientsKeep up to date with NDSS programs and referral pathwaysSupport chronic disease managementAbout YouCredentialed Diabetes Educator (CDE) status with ADEA, or actively working towards itCurrent AHPRA registration or relevant qualificationGenuine patience and a knack for explaining complex things simplyHOW TO APPLYSend your resume and a short cover note to ******.
