Across Australia Just Better Care provides in-home aged care and disability support services, supporting people to live independently.
For over 15 years, we have been supporting older people, and people living with disability, to live independently in their own homes and stay connected to their local community.

Duties and responsibilities include:
Coordinating and monitoring care plans in accordance with medical practitioner directions and support plans
Documenting client assessments, progress notes and care outcomes via digital systems and mobile devices
Liaising with allied health professionals, hospital discharge teams and community stakeholders
Monitoring and communicating on customer wellbeing and reporting on progress
Developing connections with external organisations, hospitals and community partners
Ensuring all services meet Just Better Care's policy and procedures, legislative requirements and program-specific guidelines (e.g. DVA Community Nursing)
Conducting clinical skills assessments of Community Support Professionals performing complex care services
Demonstrating commitment to best practice and evidence of continued professional development
Participating in staff meetings and attending training opportunities as required
About you:
You are an experienced RN ready to shift into a coordination or case management-style position
You enjoy clinical documentation, oversight and quality assurance as much as hands-on care
You have experience supervising or mentoring junior nursing or support staff
Caring, compassionate and friendly with strong interpersonal skills
Demonstrated high-level verbal and written communication skills
Proven relationship management with internal and external stakeholders
Strong organisational and time management skills
Experience in care coordination, case management or clinical documentation
Comfortable using technology (mobile devices) for accessing rosters, recording assessments and documenting feedback
Reliable, dependable and able to work independently and as part of a team
You thrive in a community setting with variety, autonomy and flexibility
What you will need:
Current Registered Nurse registration with AHPRA
Evidence of continued professional development
NDIS Worker Screening Clearance / Criminal Record Check
Current CPR / First Aid Certificate
Current Australian Driver's Licence
Reliable, registered and insured vehicle (comprehensive insurance preferred)
Previous experience in community care, health or aged care sector (preferred)
Proof of COVID-19 vaccination (where relevant)
