NSW Health in Western Sydney is seeking a Staff Specialist for Hospital in The Home (HiTH) under AGES.
The fixed-term 1.0 FTE position will work within HiTH and community geriatric outreach, developing district-wide integrated care and referral pathways.
The successful candidate will collaborate with Geriatric Medicine departments, lead service change, and provide high-quality, patient-centred care in non-traditional settings across district healthcare.
