About the role This position is responsible for delivering best practice support to individuals and groups across Community Care focused on people over 55 years, people with disabilities and carers.
In this role you will: Deliver flexible, participant‐centred support across individual and group activities to enhance health, wellbeing, and social connection, while meeting KPIs Manage and coordinate caseload, including assessment, planning, service delivery, referrals, and exits Facilitate group activities in an enabling way, empowering participants to take ownership and reduce reliance on formal supports Support and guide volunteers to deliver quality services Manage and resolve day‐to‐day issues, escalating where required What's on offer We are offering a full time permanent opportunity with a competitive remuneration package from $73,****** to $80,****** plus 12% superannuation .

You will also bring the following skills: Delivering best practice support to individuals and groups focused on participants achievement of goals Self-motivated with the ability to work independently, and as part of a team Ability to meet strict deadlines and work under pressure Ability to source and support people into valued community networks and activities Intermediate computer literacy including Microsoft suite of products and other applications.
