About the Opportunity

The Complex Care and Frequent User Management (CCFUM) Program is a statewide clinical initiative aimed at improving outcomes for patients with chronic and complex health conditions, including those who frequently present to NSW Ambulance.

The CNC2 Supportive Care is a senior clinician with advanced expertise in one or more of the following areas: palliative care, aged care, and chronic and complex care. This role is responsible for the strategic design, implementation, and evaluation of a supportive care model within NSW Ambulance that identifies and addresses the support requirements of older patients with complex healthcare needs and progressive life limiting illness that frequently present to NSW Ambulance.

This role plays a critical part in enhancing the quality, safety, and efficiency of care for an older vulnerable patient cohort, while supporting broader system integration and service optimisation across NSW Ambulance.

This position is for a period up to twelve (12) months with possible extension.

About You

- Registered Nurse with current Authority to Practice with at least five years post graduate nursing/midwifery applied in a mixed clinical environment and/or across a range of services.
- Postgraduate nursing qualifications relevant to the speciality field, or evidence of working towards same or other qualifications or experience deemed appropriate.
- At least 3 years full time equivalent experience working in the specialty fields of either palliative care, public acute, subacute, outpatient or community aged care, and/or chronic and complex care
- Experience working with psycho-socially vulnerable individuals who experience challenges engaging with health care providers.
- Demonstrated understanding and experience of models of supportive care as they relate to patient outreach.

Salary Range: $135,826 per annum - $138,534 per annum (Clinical Nurse Consultant Grade 2)
