IPC Health is seeking a Diabetes Educator to work with clients and families to improve health outcomes. You will deliver diabetes assessment, education, therapeutic management and connect clients to support services within a social model of health.

You will collaborate with multidisciplinary teams, maintain documentation and participate in quality improvement activities. Parental leave replacement, fixed-term until 30 June 2027, with adaptable site work and onsite parking.
