This role involves coordinating support services for individuals with disabilities, ensuring their well-being and inclusion in the community. Responsibilities include developing support plans, liaising with service providers, and advocating for clients.

Work-life balance Task & responsibilities

- Conduct intake interviews and comprehensive assessments of clients’ social, emotional, financial, and housing needs.
- Develop support plans for individuals with disabilities
- Liaise with service providers to coordinate support services
- Advocate for clients to ensure their needs are met
- Monitor and evaluate the effectiveness of support services




- Provide guidance and support to clients and their families

Qualification & experience

- Relevant in disability services or related field
- Minimum 1 year Experience working with individuals with disabilities
- Excellent communication and interpersonal skills
- Ability to work effectively in a team
- Understanding of disability rights and advocacy
- Hold a current, unrestricted driver's licence.
- Ensure you have up-to-date immunisations as required Completed certifications and checks such as a
- First Aid Certificate, Working with Children Check, Police Check and/or NDIS Worker Screening Check
