SSCS WA is a leading provider of disability support coordination services and complex clinical case management, specialising in supporting individuals with complex clinical needs, justice interface and psychosocial disabilities. We have a large clientele base in metro and regional Western Australia and are passionate about ensuring that individuals across Australia receive gold standard of care and support without compromise.
We are looking for an experienced Specialist Support Coordinator to join our team.
What You'll Do:
- Work closely with individuals with complex disabilities and their families to understand their needs, goals, and preferences.
- Use your clinical expertise to assess needs and coordinate services.
- Provide comprehensive support in navigating the NDIS system, including plan development, implementation, and review.
- Conduct thorough assessments to identify strengths, challenges, and support requirements.
- Collaborate with external service providers, community organisations, and government agencies to coordinate and access appropriate supports and services.
- Empower clients to build their capacity and support their rights and preferences within the NDIS framework.
- Maintain accurate records and documentation in accordance with NDIS guidelines and organisational policies.
- Stay informed about changes and updates to the NDIS framework and relevant legislation.
What We're Looking For:
- Qualified physiotherapist, occupational therapist, registered nurse or social worker with current registration.
- Strong communication and interpersonal skills, with the ability to build rapport and establish trust with clients and stakeholders.
- Excellent organisational and time management skills, with the ability to prioritise tasks and manage competing priorities effectively.
- Commitment to upholding the rights, dignity, and independence of individuals with disabilities.
- Interest in complex clinical case management and willingness to learn.
- Current driver's license and access to a reliable vehicle.
