In this role, you will deliver high-quality, person-centred assessments, working directly with participants to identify and articulate their support needs in a transparent, evidence-based and respectful manner.Key ResponsibilitiesConduct support needs assessments with participants and their representativesAnalyse information and external evidence to support sound, evidence-based decisionsPrepare clear, accurate assessment reportsCommunicate assessment processes and outcomes clearly (virtual and face-to-face)Apply relevant policies, procedures and legislative frameworksIdentify and escape complex or high-risk mattersCollaborate with internal stakeholders to support participant outcomesMaintain accurate records using case management systemsAdapt assessment approaches to suit individual participant needsAbout YouYou will bring:Strong communication and stakeholder engagement skillsExperience in assessment, interviewing or case managementAbility to interpret evidence and provide clear,
