About the Role:
We are seeking experienced OTs/ Physios and RNs to deliver comprehensive aged care assessments in Hobart.
These are hybrid roles combining half telehealth and half face-to-face assessments, offering variety and flexibility.
You will conduct comprehensive assessments to determine the care and support needs of older Australians.
Working as part of a multidisciplinary team, you will undertake holistic assessments considering clients' health, functional capacity, social circumstances and overall wellbeing, helping them access appropriate aged care services and supports.
Conduct comprehensive aged care assessments across both telehealth and face-to-face settings, in accordance with relevant assessment frameworks and guidelines
Assess clients' clinical, functional, cognitive, psychosocial and environmental needs
Develop recommendations and support plans to facilitate access to appropriate aged care services
Complete detailed assessment documentation and reports
Work collaboratively with multidisciplinary clinicians and stakeholders
Benefits:
$90,000 - $105,000 per annum + superannuation
5 positions available

Full-time or part-time opportunities available (minimum 4 days per week)

About You:
Relevant degree in Occupational Therapy, Physiotherapy or Nursing
Current AHPRA registration
Strong communication and clinical assessment skills
Experience in aged care, community health or rehabilitation highly regarded
Ability to work autonomously across both remote and community settings
Excellent documentation and stakeholder communication skills
Current driver's licence and own vehicle
Reliable internet connection and suitable home working setup
Eligible to work in Australia
