Always.We are St Vincent's Health Australia, Australia's largest not-for-profit provider of health and aged care services.Founded by The Sisters of Charity in ****, our +30,000 healthcare superheroes operate public and private hospitals, residential aged care, community and virtual care, and outreach programs.About the roleDeliver comprehensive social work services to patients, families and carers in the medical/surgical wards at St Vincent's Hospital, providing high-quality psychosocial care within a multidisciplinary team, and participate in the 24-hour on-call social work roster as requiredConduct psychosocial and risk assessments, contributing to secure discharge planning including community referrals and accommodation in supported living and residential aged care facilities.Provide counselling and therapeutic support to individuals and families, assisting adjustment to medical conditions and lifestyle changesAdvocate for patients and carers, including preparation of applications to NCAT (Guardianship Division) and the NDIA (NDIS) where requiredFacilitate family conferences and actively participate in multidisciplinary meetings to support collaborative, patient-centred careParticipate in supervision, quality improvement, education and research initiatives.About youHold a degree in Social Work and be eligible for membership with the Australian Association of Social Workers (AASW), with less than 9 months post graduate qualifying experience (at time of application)Be able to, under direct and indirect supervision,



conduct psychosocial assessments for patients and assess issues of risk.
Understanding of crisis intervention, short-term solution-focused approaches, and grief and loss support.Participate in multi-disciplinary meetings for patient care and safe discharge planning including referral to community services, short- and long-term accommodation/ placement for patients.Excellent time management and prioritisation abilities, alongside proficient computer literacy and strong written and verbal communication skills.Participate in supervision, professional development and quality projects.
Complete documentation according to professional standards, provide clinical handovers and maintain statistical records.
Develop and maintain resources pertaining to clinical practice.Support family conferencing to enable information exchange between the multi- disciplinary team and patients, families and carers.Willingness and ability to participate on the 24 hour, 7 day a week on-call service as required.Why work at St Vincent's?
