? Position: Community Worker ? Location: Beechboro, WA ? Salary: $80,000 – $90,000 per annum ? ABOUT THE ROLE As a Community Worker, you will provide person-centred support to individuals, helping them achieve greater independence, wellbeing and meaningful participation in the community.




KEY RESPONSIBILITIES • Provide personalised care and support• Assist with daily living activities • Support community participation and social inclusion • Encourage independence and personal development • Build positive relationships with participants • Follow individual support plans and goals • Maintain accurate client records • Work collaboratively with families and support professionals ?
