Must be available for two evenings per week, working until 7:00 pm
30-day notice required for cancellations
Productivity:

Requirements:
Must have a minimum of three months of outpatient experience or one year of physical therapy experience in other settings post-clinicals
Completion of three HealthStream Modules is required prior to assignment, with successful completion and proof of at least one shift worked necessary for reimbursement to the agency
