About the opportunity
With our organisation's expansion, we are excited to offer an opportunity for an experienced Coordinator – Individualised Services to become part of our current team at the leading disability support organisation in the Goldfields region of Esperance.
This position is offered on a Full time basis.
Within this role you will be responsible for managing a range of support services to assist people with disabilities to live the "good life".
Key responsibilities include:
Ensure positive relationships are developed and maintained with the people we support and the relevant stakeholders in their life in order to implement and continuously improve their individualised support services.
Encourage and promote choice and control for the people we support




Facilitate opportunities for people to be valued members of their community whilst ensuring strategies within their Individual Plan are aligned with their goals and aspirations to achieve their desired outcomes
Provide leadership and mentoring to support staff who come from varied cultural backgrounds with a range of different life experiences.
Essential skills and experience
Well-developed advocacy skills
Ability to build and nurture relationships at a personal, organisational and community level
Effectively manage and implement medically developed care plan requirements as directed by qualified professionals and specialists
Demonstrated computer skills including working knowledge of databases, Microsoft Excel and Microsoft Word
