For over 30 years, Northern District Community Health (NDCH) has supported healthy, inclusive rural communities across all ages.
Partnering with health services and government, we provide integrated, accessible care tailored to rural needs.
Our services include medical care, allied health, nursing, mental health, counselling, health promotion, and community programs.
We also offer in-home support for older adults to help them age in place and stay connected.
Serving the Gannawarra, Buloke, Loddon Shires, and the Rural City of Swan Hill, NDCH employs over 120 staff and 130 volunteers across towns like Cohuna, Kerang, Boort, and Swan Hill.

ROLE OVERVIEW
This role delivers high quality, person-centred personal care and practical support to clients, assisting with daily living activities such as hygiene, grooming, and self-care.
The aim is to help clients live safely, comfortably, and with dignity in their own homes.
The role also involves providing time limited, goal-oriented services that align with wellness and supporting clients to regain or maintain their independence.
KEY RESPONSIBILITIES
Travel between clients' homes to deliver care and support
Assist with personal care tasks (e.g. bathing, grooming, dressing, toileting, mobility)
Support daily living activities (e.g. cleaning, laundry, meal prep, shopping, pet care, appointments)
Promote independence through wellness
Prompt clients to take self-administered medications
Provide flexible respite care and community access support
Respect and support clients' individual, cultural, and spiritual needs
Encourage choice, dignity, and control in daily life
Work within a multidisciplinary team, reporting changes in client condition as required
KEY SELECTION CRITERIA
Essential:
Certificate III/IV in Individual Support (Aged Care/Disability) or equivalent
Current First Aid, CPR,
Manual Handling, and Food Handling certification
Experience in home care or domestic support preferred
Desirable:
Knowledge of WHS practices and ability to identify and manage risks
Commitment to Aged Care Quality and Safety standards
Strong time management and flexibility to work varied hours and locations
Excellent communication, teamwork, and conflict resolution skills
Confident using digital systems and quick to learn new technologies
