The Nurse Navigator Services team delivers flexible, patient‐centred care for adults living with chronic and complex health conditions.
Working across multidisciplinary teams and healthcare settings, the service coordinates integrated care, improves access to healthcare, reduces fragmentation and supports better health outcomes across THHS.

Join the Nurse Navigator Services team as a Nurse Navigator (Complex Chronic Diseases – Adult).
In this advanced practice role, you will provide patient‐centred care for adults living with chronic and complex conditions, working collaboratively with multidisciplinary teams to improve healthcare access, streamline patient journeys and achieve better health outcomes through integrated care.
Requirements
Mandatory qualifications: Bachelor of Nursing or equivalent and proof of registration (AHPRA).
This is a Vaccine Preventable Disease (VPD) risk role.
Probation period may apply.
Pre‐employment checks will be conducted.
