We are seeking an experienced and patient-focused Medical Practitioner to join our Transition Care Service, providing sub-acute care across our Adelaide and Walkerville sites.

Working within a supportive multidisciplinary team, you will play a key role in delivering high-quality, person-centred care. You will collaborate closely with nursing, allied health and care teams to support positive health outcomes and enhance patient wellbeing.

- Day shifts only | 8:00am – 4:30pm across a 7-day roster

/n

- Convenient metropolitan locations in Adelaide and Walkerville with in office parking

/n

- Collaborative and supportive multidisciplinary team focused on exceptional patient experience and outcomes

About You

/n

/n

- Have full unrestricted AHPRA registration with a minimum of five years post-registration clinical experience.

/n

- Postgraduate medical specialist qualifications will be viewed highly favourably.

/n

- The ability to work confidently and effectively within a sub-acute setting (bridging the gap between primary and acute healthcare).

/n

- Strong communication and collaboration skills, with a commitment to multidisciplinary care.

/n

- Hold a valid South Australian "Working with Children Check" and a National Police Clearance or are willing and eligible to obtain.

/n

- If successful, willing and able to provide proof of vaccinations.
