Bethanie is a respected Australian aged-care provider seeking a Case Manager for our Home Care team in Western Australia.
You will coordinate services for a portfolio of clients, using a wellness and reablement approach to support independence and protected, confident living at home.
You'll work with Clinical Nurses, Allied Health specialists and Support Workers in a multidisciplinary team, balancing client visits, work from home and one day per week onsite at our Port Kennedy office.
