Job responsibilities:

- Work 36 hours per week with 12-hour shifts.
- Document care delivered, patient response, and communications in the medical record.
- Coordinate scheduling, referrals, and follow-up care with clinical and administrative teams.
- Perform medication reconciliation activities as directed and communicate discrepancies.
- Educate patients on treatment plans, follow-up instructions, and self-management strategies.
