Salary: $95,000 - $115,000 per year (pro rata) + 12% super

About the Role

MTHC is seeking a Care Navigator to join our Shepparton Medical Centre team. Based in Shepparton, you will support patients with chronic and complex health needs across the greater Shepparton region by coordinating care, connecting them with appropriate services and strengthening continuity of care.

You will:

Support patients from identification and assessment through to care planning, service linkage, monitoring, and planned discharge

Work collaboratively with GPs, nurses, allied health professionals and health service providers to support coordinated and multidisciplinary care.

Develop holistic, goal-oriented care plans with patients and coordinate referrals and connections to appropriate health services

Monitor patient progress and escape clinical risks where required

Strengthen referral pathways across Greater Shepparton and contribute to service development, evaluation and improvement of integrated care models

Provide clinical supervision, guidance and assessment for students undertaking placement within the Care Navigation Program, working collaboratively with the nursing team to support day to day placement learning and development.

This role will suit a clinician or health professional passionate about improving healthcare access, navigating complex health systems, developing integrated models of care and supporting the development of future health professionals.

Part-time opportunity – 0.4 FTE across 2 days per week, providing an opportunity to complement other clinical, professional or personal commitments. Working days can be negotiated.

What You’ll Bring

Relevant tertiary qualification in Nursing, Allied Health, Social Work, or related discipline

Current AHPRA registration (if applicable)

Demonstrated experience in care coordination, chronic disease management, or community-based health services

Strong knowledge of, or demonstrated ability to rapidly develop knowledge of Victorian primary care, hospital, and community service systems Demonstrated ability to work with vulnerable and complex patient cohorts

Demonstrated experience or capability in supervising, mentoring and supporting students or developing health professionals in a clinical, health or community setting

Excellent interpersonal and communication skills with the ability to build effective relationships across multidisciplinary teams

Experience working within multidisciplinary and interprofessional environments

Valid Australian Driver’s Licence

Desirable (not essential — we'd still love to hear from you without these)

Experience working within community health or general practice or teaching health clinic settings

Knowledge of Medicare-funded services and pathways such as the GP Chronic Disease Management Plan

Experience working with priority populations including culturally diverse communities, Aboriginal and Torres Strait Islander peoples, aged care residents, and socially isolated patients

Experience in quality improvement, reporting, and data collection aligned with funded KPIs

About Shepparton Medial Centre

Shepparton Medial Centre provides primary healthcare to the diverse community within Greater Shepparton. Our multidisciplinary workplace brings together general practice, nursing and other health professionals to provide coordinated, person-centred care.

As a teaching health clinic, SMC also supports the education and development of future health professionals. The Care Navigator will combine patient care and service navigation with clinical supervision of students undertaking placements within the program.
