Description
As our Care Coordinator, you will:
- Understand the Commonwealth Home Care Support and HACC programs
- Conduct assessments: including identifying any health risks, medical conditions, or social and environmental factors
- Develop care plans
- Coordinate care
- Provide support and education.
Skills and Experiences
What we're looking for
- Current Australian driver's licence
- National police check
- Working with vulnerable people/children check
- Minimum Certificate III in a relevant discipline
