Multicultural Care is a leading not-for-profit organisation providing culturally sensitive home and community-based support to older people, people with disabilities, and their carers. We are seeking an experienced and values-driven Team Leader, Care Partner who is responsible for the provisioning of high-quality case management, support planning, and care coordination services across all home care program types (including SAH and CHSP).

The position reports to the Manager Care Services and has an key role in providing support to the management team to ensure all care partnership activities is of primary focus to achieve set KRAs, KPIs and meets relevant compliance standards.

The Team

Leader is responsible for supervising the day-to-day operations of the Care Partnership team which provides care partnership, case coordination and care management services to all clients at Multicultural Care.

Key Responsibility Areas: Assist the Manager Care Services (MCS) in developing and delivering the growth strategy for MC’s Home Care (Aged and Disability) programs Effective front line management of all care partners, and associated staff to successfully deliver against pre-determined guidelines and targets Ensure effective efficient resource allocation, including case load management, clientele risk management, and care partnership billable and productive hours. Ensuring daily practices meets relevant compliance and regulatory standards, as well as organisational policies and procedures. Provide effective frontline management to the team to ensure best practices across all areas of operations, including professional development and training, and meeting of individual and team, KPIs and KRAs as set by the MCS.





Contribute to the continued development of the Aged and Disability programs’ clientele, including a continuum of care for clients through the different levels of the their relevant funding Ensuring appropriate induction, training and ongoing support and supervision for all Care Partnership staff Preparation of accurate and timely reports on related service area metrics and outcomes Comply with all MC Quality policies, procedures and relevant legislation Contribute to continual improvement process’ and systems Protect sensitive information and privacy of both colleagues and clients Lead the meeting of relevant quality standards for the relevant areas/programs Contribute to the wider quality framework and development Other relevant responsibilities as deemed necessary by the MCS Skills, Experience and Competencies Essential Minimum 5 years of senior case management experience within the Community Care sector In-depth understanding of key Aged Care and Disability program types and funding (ie. SAH, CHSP, NDIS) Demonstrated experience in managing a team (minimum at a front line team lead level) A sound understanding and experience in service review and development Demonstrated experience in managing CRM system (Alayacare and other associated systems) Demonstrated proficiency in the use of a wide range of IT applications Demonstrated commitment to a customer focused culture and ensuring quality outcomes for clients Demonstrated knowledge of continuous quality improvement Working knowledge and experience in relation to contemporary issues, legislation and standards impacting Home Care Ability to motivate, develop and provide feedback to staff/ teams Demonstrated experience in executing and managing change Current driver license and car Mandatory Requirements Pre-employment Valid National Police check NDIS Worker Screening Check Up to date COVID-19 Vaccination
