ITACS provides a time-limited, goal-oriented service for older people who would benefit from additional low intensity therapy in their own homes as well as assisting with the decision-making process around long term care service supports. As a Registered Nurse in ITACS you will provide a comprehensive nursing service including assessment and interventions, care planning, the delivery of nursing treatment e.g. wound care in the client's home, medication management and case management.

You will also be required to liaise with external stakeholders such as GP's and service providers. The position functions as part of a multidisciplinary team with a teamwork approach to the provision of low intensity home-based therapy programs to frail elderly clients who are ready for discharge from sub-acute hospital care but are unable to return home without additional care and support.
