Your Role: As a Registered Practice Nurse, your responsibilities will include:

- Conducting thorough patient assessments and diagnoses.





- Prescribing medications and recommending lifestyle changes to effectively manage chronic conditions.

- Ordering and analyzing pathology and radiological tests.

- Educating and supporting patients with chronic diseases through tailored Individual Care Plans.

- Promoting health literacy and patient engagement within the community.
