The Chance

We're seeking a dynamic and relationship-focused person to help improve health outcomes across the Murray PHN region. This role combines project coordination, stakeholder engagement and system improvement, offering the opportunity to influence service delivery, support innovative models of care and strengthen collaboration across the health sector.

Key Accountabilities

- Coordinate the development, review and promotion of HealthPathways, working closely with clinical subject matter experts and key stakeholders to support regional health priorities.
- Build and maintain robust relationships across the health sector, leading stakeholder engagement activities that support knowledge sharing, collaboration and pathway development.
- Identify and facilitate service improvement opportunities, contributing to the implementation of commissioning projects, new service models and system redesign initiatives.
- Lead and coordinate clinical working groups, documenting outcomes, identifying barriers and enablers, and supporting evidence-based recommendations for improvement.
- Act as the primary point of contact for HealthPathways, providing project leadership, supporting provider uptake and contributing to collaborative, innovative ways of working across the organisation.

About You

You are a proactive and organised professional who enjoys bringing people together to achieve meaningful outcomes. With strong stakeholder engagement and project coordination skills, you can build trusted relationships, facilitate collaboration and translate complex information into practical solutions that support healthcare improvement.

Key Capabilities

- Relevant degree qualification in a health-related field and/or approximately five years in position with similar responsibilities and experience.
- Strong stakeholder engagement and project coordination skills
- Understanding of the primary health care sector, its systems, practices,



and emerging trends.
- Data analysis and reporting skills with a record of accomplishment of using data to drive informed decisions.
- A connection with the Murray PHN catchment, our values and vision.
