Community Carers NSW is seeking an experienced and motivated Service Coordinator to join our growing team at our Caringbah Head Office.

This role will support NDIS participants across the Sutherland Shire and surrounding areas, ensuring they receive high-quality, person-centred services that support their goals, wellbeing and independence.

About the Role

As a Service Coordinator, you will be responsible for coordinating supports and building strong relationships with participants, families, support workers and other key stakeholders.

Key Responsibilities

- Coordinate NDIS supports and services in line with participant goals and needs.
- Build and maintain strong relationships with participants, families, support workers and allied health professionals.
- Conduct participant reviews and monitor service delivery and outcomes.
- Assist with onboarding current participants and act as a key point of contact.
- Maintain accurate participant records, case notes and documentation.
- Liaise with internal teams to support effective scheduling and service delivery.
- Identify and resolve service-related issues promptly and professionally.
- Support participants to access community, mainstream and funded supports.
- Ensure services are delivered in accordance with NDIS Practice Standards and organisational policies.

About You To be successful in this role, you will have:

- Minimum 2 years' experience in NDIS Service Coordination, Support Coordination, Case Management or a similar role.
- Strong experience working with NDIS participants and an understanding of the NDIS framework.
- Excellent communication, relationship-building and problem-solving skills.




- Strong organisational skills and the ability to manage competing priorities.
- High attention to detail and experience maintaining accurate records.
- Proficiency with Microsoft Office and client management systems.
- The ability to work independently while contributing positively to a supportive team environment.
- Level 2 or Level 3 Support Coordination experience is highly regarded.

Essential Requirements

- Current NDIS Worker Screening Check.
- Current Working With Children Check.
- Current First Aid & CPR.
- Current Driver Licence and reliable vehicle.
- National Police Check or willingness to obtain.
- Full working rights in Australia.
