Registered/ Enrolled Nurse
Rural Health Tasmania have an exciting opportunity for a nurse to join our Rural Primary Health Service. The role is part time .8FTE based out of Rural Health Tasmania's Smithton and Wynyard offices The nurse will provide person centred support to community members experiencing chronic health conditions and mental health concerns. Working with clients, the nurse will coordinate care, connect clients with appropriate services and support them to improve their health and wellbeing, in a stepped care model.
With a primary focus on Chronic conditions and comorbidities including but not limited to Cancer, Mental Illness, Circulatory Disease including CVD, Respiratory Disease including COPD, Musculoskeletal conditions, and Injury:
The nurses key responsibilities include:
- Coordinating care with GPs, psychiatrists, mental health services and allied health professionals.
- Assessing client needs, monitoring progress and supporting recovery and self-management.
- Providing education and support around medications, chronic conditions and mental health.
- Making appropriate referrals to counselling, psychological services and other supports.
- Advocating for clients and improving access to coordinated, culturally appropriate care.
- Supporting improved health outcomes and reducing preventable hospitalisations.
- Maintaining high standards of clinical documentation, data collection and service quality.
- Participating in career development and required training.

Selection Criteria;
- Relevant nursing qualification
- Minimum 2 years’ experience in clinical or community settings; Care coordination and/or case management experience
- Current registration with APHRA
- Evidence of communication skills
- Highly organised with ability to keep accurate notes and records
- Sound understanding of the social detriments of health
- Experience with health IT systems and Microsoft suite of programs
- Current unsuspended Driving Licence
- Current Working with Vulnerable Persons registration (Employment Class)
- Current (within 3 months) National Police Check or ability to get one.
