Mercy Health in Victoria is seeking a Senior Social Worker/Care Coordinator for the Health Independence Program (HIP) based at Werribee Mercy Hospital. You’ll join a multidisciplinary team to coordinate care for HIP clients in the Wyndham catchment on weekdays.
You will conduct psychosocial assessments, support access to Centrelink/NDIS/VCAT, and lead care coordination and supervision within the outpatient and community settings. Commitment to patient-centred care is essential.
