About Community Care Team

Community Care Team is a warm, family-focused organisation dedicated to empowering individuals with disabilities. We believe in independence, choice, and meaningful outcomes - supported by a team-oriented and inclusive culture that values your skilled growth and work-life balance.

About the Role

We are expanding our Positive Behaviour Support team and seeking passionate practitioners at all levels - from current graduates to senior clinicians.

In this role, you’ll design and deliver evidence-based Positive Behaviour Support (PBS) Plans that enhance wellbeing and independence for NDIS participants. You’ll collaborate with participants, families, and multidisciplinary teams to reduce restrictive practices and build capacity through education and support.

Key Responsibilities

Conduct Functional Behaviour Assessments (FBAs) and develop individualised PBS Plans.

Implement strategies that promote skill development and reduce behaviours of concern.

Provide training and guidance to families, carers, and support staff.

Monitor progress using data and participant feedback, adjusting plans as required.

Ensure all practice aligns with NDIS Quality & Safeguards Commission requirements.

What We’re Looking For

We welcome applications from both experienced practitioners and emerging graduates.

You may be:

Eligible for NDIS Junior Behaviour Support Practitioner registration, or already deemed suitable at a higher level.

Tertiary-qualified in Psychology, Social Work, Occupational Therapy, Counselling, Teaching, or related fields.

Skilled in report writing, data-driven analysis, and person-centred communication.

Committed to continuous learning and evidence-based practice.

Holding a current NSW Driver's Licence and able to travel locally.

Able to provide a current National Police Check, Working with Children Check, and NDIS Worker Screening.

What We Offer
Career development and internal clinical supervision.
Supportive and collaborative multidisciplinary workplace.
Flexible work arrangements and autonomy in your schedule.
Employee Assistance Program (EAP).
The chance to make a genuine, lasting difference in people’s lives.
