Disability Connect Victoria is seeking a reliable and experienced Disability Support Worker to join our team in Moe on a permanent part time basis.
This role involves supporting a participant with daily living activities while promoting their independence, choice and wellbeing. The successful applicant will need to be confident providing manual handling support and managing health-related needs in line with the participant’s individual support plans.
Regular Roster
Thursday: 3:00 pm to 7:00 am, including a sleepover
Saturday: 7:00 am to 3:00 pm
Sunday: 7:00 am to 3:00 pm
Applicants must be available to work the regular roster listed above.
About the Role
Your responsibilities will include:
Supporting the participant with daily living activities
Providing domestic assistance and meal preparation
Completing manual handling using a hoist and sling
Supporting diabetes management in line with management plans
Supporting medication administration
Monitoring and supporting asthma management
Completing accurate progress notes and documentation
Working collaboratively with the participant’s support team
Providing sleepover support
A private vehicle is not required for this position.
About You
To be successful, you will have:
Experience using hoists and slings
Experience supporting people with diabetes
Knowledge of asthma management
Medication administration experience
Safe manual handling skills
Good cooking and household management skills
Strong communication skills
The ability to work as part of a team
Initiative and the ability to work independently
A person-centred and respectful approach
Mandatory Requirements
Certificate III in Individual Support, Disability or a related qualification
Current NDIS Worker Screening Check
Current Working with Children Check
Current First Aid and CPR certificates
NDIS Worker Orientation Module
Evidence of Australian working rights
