Nurse Practitioner Opportunities (AOD | Aged Care | Primary Care | Telehealth)
Wellness Partners Foundation – Brisbane QLD
Help build a healthcare system that reaches the people who need it most.
Across Australia, too many people still face barriers to accessing timely, coordinated and compassionate healthcare.
Whether because of addiction, homelessness, ageing, disability, financial hardship or other life circumstances, the people who need healthcare most are often the least likely to receive it.
AtWellness Partners FoundationandWellness Partners Medical, we're building a different future for healthcare.
We're building a connected, multidisciplinary model of care that brings together Nurse Practitioners, General Practitioners, Medical Specialists, Registered Nurses, Pharmacists and community partners to deliver healthcare where people already feel safe, trusted and supported.
Our services extend acrossprimary care, aged care, alcohol and other drugs (AOD), homelessness, justice and transition settings, family violence services, community outreach, telehealth and other priority population environments, ensuring healthcare reaches people where it can make the greatest difference.
As a registered charity led by healthcare professionals, we're a mission-driven organisation dedicated to improving health equity through collaboration, innovation and person-centred care.
If you're looking for more than just another Nurse Practitioner role and want the opportunity to help shape the future of connected and compassionate healthcare, we'd love to hear from you.
