About the Role
You'll coordinate care for veterans and Medicare-funded clients living with chronic and complex health conditions.
Working via telehealth,



you'll become each client's primary point of contact throughout their care journey, ensuring they receive coordinated, person-centred support.
Key Responsibilities
Conduct comprehensive biopsychosocial assessments
Develop and coordinate individualised care plans
Participate in GP-led multidisciplinary case conferences
Deliver regular care coordination consultations and 90-day reviews
Coordinate referrals across allied health services
Identify and manage psychosocial risks
Advocate for clients and facilitate access to appropriate services
Maintain accurate clinical documentation in line with professional standards
About You
To be successful, you'll have:
Bachelor or Master's degree in Social Work
Current AASW membership (or eligibility prior to commencement)
DVA experience desirable
Strong assessment and care coordination skills
Excellent communication and rapport-building abilities
Confidence delivering services via telehealth
Ability to work independently while collaborating within a clinical team
Current National Police Check (or willingness to obtain)
