Hand in Hand Disability Support – Kallangur QLD
We are currently looking for Disability Support Workers to join our team to work with participants across North Brisbane. You will be working within a supportive and professional environment, providing meaningful assistance to people within your community to enhance their independence and quality of life. You will be part of a team-oriented team that values ongoing training, professional development, and best-practice service delivery.
We are seeking experienced Disability Support Workers who are committed, adaptable, and person-centred in their approach. Ideal candidates will demonstrate the ability to think creatively, work proactively, and support participants to achieve both their current and long-term goals. Applicants must hold a minimum Certificate III in Individual Support or Community Services and have at least one year of relevant industry experience.
Hand in Hand Disability Support delivers in-home support, Supported Independent Living (SIL), short-term accommodation, and community access services to people living with disability. Supports are provided across a seven-day rotating roster, including day, evening, overnight, active overnight, and sleepover shifts. Applicants must be flexible and available to work across a 24-hour care environment.

Duties and responsibilities may include:
Personal care: showering, dressing and grooming
Medication assistance
Supporting participants with behaviours and following a Positive Behaviour Support Plan
High Intensity supports including PEG / Enteral feeding, Ventilation Support, Complex Bowel Care, Catheter Support, Epilepsy, Subcutaneous Injections / Diabetes Support,
Complex Wound care Dysphagia Support including assistance with Mealtime Management.
Domestic assistance: light household cleaning, laundry and meal preparation.
Community Access: social activities, grocery shopping.
Assistance in travelling to medical appointments.
Supporting participants in the use of mechanical aids and personal equipment including wheelchairs, manual or electric hoists, slide sheets and moving boards.
Supporting participants who have mental health challenges.
Supporting participants with complex and/or challenging needs.
Experience with high intensity supports is preferred but not essential as training will be provided.
Expected to follow the company policies and procedures and code of conduct.
Monitoring the well-being of participants reporting and documenting on their progress.
Attending training as required.
Be asked to work anytime over a 24-hour period 7 days a week, this may include sleep over shifts or awake shifts.
Candidates must be available to work full seven-day coverage including weekends, public holidays, and across all shift types as required by the 24-hour care environment.
You will have:
Minimum Certificate III or IV Disability or Community Services OR Minimum of 1 years' experience in disability support work
Current CPR / First Aid Certificate
Current and Valid Blue Card & NDIS Worker Screening Check
Current Australian Driver's License
Reliable, Registered and Insured vehicle
Smartphone with internet access
NDIS Worker Orientation Module
Australian work rights are required for this position
