About The Role
Seeking an
Experienced Complex Case Manager
to join our team, specialising in
severe and catastrophic injury
. This role is suited to a senior clinician who thrives in managing complex medical presentations and working collaboratively with multiple stakeholders to achieve meaningful, long-term outcomes.

You will manage a portfolio of participants with significant injuries, providing expert rehabilitation coordination, strategic planning, and advocacy throughout their recovery journey.
Key Responsibilities
Lead and coordinate the rehabilitation and recovery of participants with complex and severe injuries, including Traumatic Brain Injury, Spinal Cord Injury, and Amputation
Conduct comprehensive biopsychosocial assessments to identify barriers, risks, and opportunities for recovery
Develop, implement, and review individualised, goal‐driven rehabilitation plans aligned with participant needs and scheme requirements
Provide high‐level case management, including proactive problem‐solving and escalation where required
Liaise closely with medical specialists, treating providers, community team members, support coordinators, families, and scheme agents
Monitor progress, outcomes, and service delivery, adjusting strategies to optimise recovery and independence
Prepare clear, timely, and clinically sound reports and recommendations for stakeholders
Advocate for participant needs to ensure access to appropriate supports, services, and resources
Maintain accurate, compliant, and detailed case documentation in line with legislative and organisational standards
About You
Relevant tertiary qualification in Rehabilitation Counselling, Occupational Therapy, Nursing, Social Work or Psychology
Minimum 5 years' experience working in rehabilitation or case management services for persons with significant or catastrophic injury.
Demonstrated expertise managing complex, long‐term cases with multiple stakeholders
Strong negotiation, influencing, and stakeholder engagement skills
Advanced problem‐solving and clinical reasoning abilities
Highly developed written and verbal communication skills
Sound knowledge of rehabilitation frameworks, case management principles, and relevant legislation
Exceptional organisational and time‐management skills, with the ability to work autonomously
