All Aged Care Ltd is a not-for-profit provider delivering personalised, in-home aged care services across Brisbane and South-East Queensland. We support older Australians to live independently and with dignity through culturally inclusive, person-centred care.
As part of ongoing aged care reforms and service growth, we are seeking an experienced
CHSP Service & Care Coordinator
to join our team at our Spring Hill office.
The Role
This is a practical and varied coordination role that combines client service coordination, administration, and communication. You will support CHSP clients from initial intake through to ongoing service delivery, ensuring services are appropriate, well-coordinated, and responsive to changing needs.
While this role is not responsible for full rostering, you will work closely with the rostering team and care workers to help schedule new services, communicate service requirements, and support smooth delivery of care.
You will also play a key role in onboarding new clients, preparing sign-up documentation and clearly explaining CHSP and Support at Home programs to clients and their families.
Key Responsibilities
Coordinate CHSP services in line with program guidelines and organisational policies
Manage intake and onboarding of new clients, including preparation of sign-up documents
Explain CHSP and Support at Home services clearly to new clients and families
Schedule and coordinate new services and service changes in collaboration with the rostering team
Communicate service requirements with care workers and participate in staff meetings as required
Monitor client needs, support service reviews, and assist with referrals for reassessments where appropriate




Maintain accurate client records, service notes, and compliance documentation
Provide clear and professional communication with clients, families, care workers, and internal teams
Contribute to reporting, quality activities, and general administrative tasks as needed
About You
You are organised, approachable, and comfortable balancing coordination, administration, and communication. You understand how community aged care services operate and enjoy working collaboratively with care teams.
Experience in aged care or community services (CHSP, HCP, or Support at Home preferred)
Understanding of service coordination, client onboarding, and basic scheduling processes
Robust organisational and communication skills
Confidence explaining aged care programs and services to clients and families
Proficiency with client management systems and Microsoft Office
Valid driver's licence and full Australian work rights
National Police Check (or willingness to obtain)
Experience working closely with rostering teams or direct care workers
Familiarity with My Aged Care, CHSP guidelines, or Support at Home reforms
