Clinical Care Partner Community Services Gorges Hall (On-site) Salary: $85,000 $110,000 + super Salary Packaging: Up to $15,900 tax-free + additional $2,650 per year (meals entertainment) About the Role We are seeking an experienced Registered Nurse to join our Community Services Team , supporting older people to live safely and independently at home under the Support at Home Program .
This role combines clinical care, care coordination, education, and advocacy , working across home care services, group social support programs, and seniors' activities.
You will deliver person-centred care with a strong focus on wellness, reablement, and independence .
Key Responsibilities Conduct comprehensive client assessments and develop care plans Coordinate in-home services and liaise with external providers Provide education, guidance,
