As an experienced Nurse Mentor/Clinical Care Coordinator, you will play a key role in supporting and developing a dedicated clinical team while ensuring residents receive the highest standard of person-centred care within this welcoming 75-bed coastal aged care facility.

YOU | You will hold current, unrestricted Div 1 Registered Nurse registration with AHPRA and have experience providing clinical leadership, mentoring, or coordinating care within an aged care setting.
Whether you are currently working as a Clinical Care Coordinator, Care Manager, Nurse Mentor, Clinical Nurse, Senior Registered Nurse or Team Leader, you will bring a resident-centred approach, robust clinical judgement, and a genuine passion for developing others.
• Current unrestricted AHPRA registration as a Registered Nurse.
• Demonstrated experience providing clinical leadership, mentoring or coordinating care within residential aged care.
• Previous experience as a Clinical Care Coordinator, Care Manager or Nurse Mentor is highly regarded but not essential.
• Sound understanding of aged care assessments, care planning, AN-ACC,
compliance and accreditation requirements.
• Proven ability to mentor and support RNs, ENs and PCWs while fostering a positive and collaborative team culture.
• Excellent communication, stakeholder management and organisational skills with the ability to remain calm under pressure.
• Current NDIS Worker Screening Clearance and Police Check (within 12 months).
• Available to work Monday to Friday afternoon shifts and either reside within 30 minutes of the facility or be willing to relocate.
THE ROLE | Fantastic opportunity to join one of Australia's fastest-growing aged care providers in this full-time Nurse Mentor/Clinical Care Coordinator position based within a welcoming 75-bed facility near Port Noarlunga.
Working Monday to Friday afternoon shifts, you will enjoy a leadership role focused on supporting both residents and your clinical team.
Reporting to the General Manager, with ongoing support from the Clinical Care Manager, you will provide day-to-day clinical leadership while mentoring staff and helping drive exceptional resident outcomes.
Key responsibilities include: • Mentor, coach and support Registered Nurses, Enrolled Nurses and Personal Care Workers to build clinical capability and confidence.
• Provide clinical leadership to ensure high-quality, person-centred care for all residents.
• Oversee assessments,
care planning, documentation and compliance with aged care standards.
• Coordinate multidisciplinary care with GPs, allied health professionals, hospitals and families.
• Support the management of complex clinical care, AN-ACC requirements, referrals and resident health escalations.
• Lead clinical handovers, facilitate effective team communication and contribute to quality improvement initiatives.
• Promote a positive team culture while supporting workforce planning and day-to-day clinical operations.
