We are seeking a compassionate and dedicated Family Support Worker to join their team in Revesby, New South Wales.
This role involves client support, case management, assessments, advocacy, documentation, and collaboration with a multidisciplinary team.
Key Duties and Responsibilities: ·Assist in the assessment of client and family needs to inform personalised support plans.
·Provide direct practical support, including personal care, domestic assistance and behaviour support.
·Support clients with accessing community activities, appointments and services.
·Contribute to the safety and wellbeing of children, individuals with disability and vulnerable families.
·Collaborate with case managers, behaviour support practitioners and other staff to deliver effective wrap-around services.
·Build trust and rapport with clients and their families, acting as a positive role model.
·Maintain accurate documentation and prepare written reports based on observations and client interactions.
·Participate in training sessions to learn about legislation, child protection frameworks, trauma-informed care and the NDIS.
·Adhere to organisational policies, including child safety, privacy and cultural inclusion ·Learn to identify when and how to refer families to appropriate external services when additional intervention is needed.
Requirements: Qualifications: ·Diploma (ANZSCO Skill Level
2) or at least three (3) years of relevant experience ·Qualification in Aged Care, Disability, Mental Health, or Community Services (preferred) ·Completion of the NDIS Worker Screening Check ·CPR and First Aid certification Experience: ·Minimum three (3) years of experience working in a related field.
·Strong understanding of case management, care planning, and assessment processes.
·Excellent communication, interpersonal, and advocacy skills.
·Ability to work collaboratively with a team and independently manage a caseload
