Team Facilitator – Jurien Bay Part time position Work from home and in the community Our Team Facilitators are an integral link between our Support Workers and Service Coordination teams.
They utilise team leading skills to balance direct customer support and play a valuable role in each customers journey.
This role also involves a mix of direct supports, administration and rostering duties.
Team Facilitator Duties: Developing a good understanding of the customer and their support team to plan and assist support services Coaching, mentoring and supervising support workers Reviewing and assessing services for customers Maintaining records on customers' progress and aiding in reporting back to funders, including NDIS Organising staff rosters and providing back up support Developing resources and relationships within the community Personal care, domestic assistance, mental health support Complex personal care and challenging behaviours (training and resources provided) Skills, experience checks: A valid Driver's licence and reliable vehicle (essential)
