About the role We are looking for a Social Worker who is passionate about assisting those impacted by workplace injuries.
At Ascent Rehabilitation Pty Ltd, you will play a vital role in helping individuals recover from injuries, illnesses or disabilities.
This full-time position, based in Canberra ACT ****, involves case management activities including conducting comprehensive biopsychosocial assessments; developing and implementing tailored rehabilitation plans to support our clients' recovery and reintegration into their daily lives; and coordinating clients access to services and supports.
This is for an entry level role, no industry experience is required - new graduates are encouraged to apply!
What you'll be doing Conducting comprehensive biopsychosocial assessments to evaluate clients' needs.
Collaborating with clients, their families and healthcare professionals to create customised recovery and rehabilitation plans tailored to their individual circumstances and needs Empowering clients to address barriers in their recovery and return to meaningful occupations Advocating for solutions and liaising with relevant stakeholders to ensure a seamless rehabilitation process Providing education and support to clients throughout the rehabilitation journey Continuously developing your professional knowledge and skills to deliver high-quality,
