Complex Communities Foundation Ltd is seeking compassionate, reliable and experienced Support Workers to provide person-centred assistance to people with disability and complex support needs.
The successful candidate will support participants to live safely, independently and confidently while promoting dignity, choice, inclusion and community participation.
Key Responsibilities Provide respectful, person-centred support in accordance with each participant's individual care plan.
Assist participants with personal care, hygiene, dressing, grooming and daily living activities.
Support participants with meal preparation, medication assistance and household tasks.
Assist with community access, appointments, shopping, recreational activities and social participation.
Encourage participants to develop independence, confidence and everyday living skills.
Provide secure transport where required.
Maintain accurate progress notes, incident reports and other service documentation.
Identify and report changes in a participant's health, behaviour or support needs.
Follow workplace health and safety, infection-control and safeguarding procedures.
Work professionally with participants, families, guardians, healthcare professionals and other team members.
Essential Requirements Certificate III or IV in Individual Support, Disability, Community Services or a related qualification is desirable.
Previous experience supporting people with disability or complex needs is preferred.
Current NDIS Worker Screening Check.
Current National Police Clearance.
Working with Children Check, where required.
Current First Aid and CPR certificates.
Valid Australian driver's licence and access to a reliable, insured vehicle.
Understanding of person-centred practice, dignity of risk and participant choice.
Good communication, reporting and interpersonal skills.
Ability to work independently and as part of a team.
Availability to work flexible shifts, including weekends, evenings and public holidays.
What We Offer Annual salary between $75,000 and $90,000 , depending on qualifications and experience.
