We're seeking a compassionate and skilled Clinical Care Coordinator in Hervey Bay to manage a clinical caseload, lead risk assessments, and coordinate holistic, person-centred care for clients with severe mental health conditions across the CCQ region.
Working closely with a multidisciplinary team, you'll independently support smooth transitions between services, drive quality improvement, and contribute to positive client outcomes through ethical, collaborative practice.
