You'll shape hospital avoidance strategies, support secure discharge pathways and build strong partnerships that improve patient flow and outcomes across CALHN.
• Lead hospital avoidance and supported discharge initiatives • Build rapid assessment pathways across care settings • Coordinate safe, timely discharge for complex patients • Partner with health and community services • Guide best practice, education and service improvement About You You're a collaborative nursing leader who thrives in complex environments and is passionate about improving patient care beyond the hospital.
