Role Overview As a Registered Nurse specialising as a Practice Nurse, you will provide comprehensive clinical care, lead medication management, and support chronic disease management programs.
You will be an integral member of the care team, guiding junior staff and liaising with residents, families, and allied health professionals to deliver individualised care plans.
Key Responsibilities Provide high-quality nursing assessments, wound care, and clinical interventions aligned with best practice.
Administer and manage medications, maintain accurate documentation and follow medication safety protocols.
Coordinate and implement chronic disease management and health promotion initiatives for residents.
Supervise and mentor enrolled nurses and support staff, fostering professional development and clinical competence.
Work collaboratively with GPs, allied health,



pharmacy and external providers to optimise resident outcomes.
Contribute to care planning, incident reporting, quality improvement activities and accreditation readiness.

Essential Qualifications and Experience Current AHPRA registration as a Registered Nurse.
Experience in aged care or community nursing; candidates with 1–10 years of relevant experience are encouraged to apply.
Clinical competency in medication administration, wound management and chronic disease care.
Strong communication skills, clinical judgement and the ability to lead a small clinical team.
Current CPR and immunisation certifications, and clear police and working with vulnerable people checks as required.
