Company: Fraser Coast Disability Services Pty Ltd Position: Residential Care Officer Salary: $75,000 – $85,000 plus Superannuation Term: Permanent, Full-time Location: QLD About Us Fraser Coast Disability Services is a family-owned and locally operated NDIS provider supporting participants across Hervey Bay, Maryborough and the wider Fraser Coast.

Our services include individual supports, supported independent living, respite, community access, and programs and workshops.

Key Responsibilities Assess clients' individual needs and plan, develop and implement appropriate care, training and support programs within residential and supported accommodation settings.
Interview clients and consult with their families or representatives to identify the nature and extent of their personal, social, behavioural and practical support needs.
Develop, implement and review individual support plans in accordance with clients' assessed needs, NDIS goals and relevant care requirements.
Monitor clients' progress and wellbeing, evaluate the effectiveness of support programs and make recommendations for adjustments where required.
Coordinate the delivery of residential care and support services to ensure clients receive appropriate, consistent and person-centred assistance.
Assist clients to develop practical life skills and strategies to manage social, emotional and financial challenges and increase their independence.
Support families and carers by providing information, guidance and assistance in relation to clients' care and support needs.
Identify and assess available community resources relating to health, welfare, housing, employment, education, training and other relevant services.
Refer clients to external agencies,



health professionals and community organisations for additional or specialised assistance.
Liaise with families, community groups, welfare agencies, government bodies and other service providers to coordinate services and promote awareness of available community supports.
Monitor clients' safety and wellbeing within residential facilities and supported accommodation, including compliance with individual care and behaviour support plans.
Prepare and maintain accurate case notes, progress reports, support-plan reviews and submissions for funding and resources in accordance with organisational and regulatory requirements.
Contribute to the ongoing review and improvement of residential care programs, service-delivery practices and client outcomes.
Qualifications and Experience A relevant qualification in community services, disability support, aged care, or a related field.
At least three years of relevant experience may substitute for the formal qualifications.
Experience working in residential care, supported accommodation, or community-based disability services.
Sound understanding of person-centred care principles and the NDIS framework.
Strong organisational and time management skills, with the ability to manage multiple client needs and documentation requirements.
Proficiency in completing case notes, progress reports, and care-related documentation accurately and efficiently.
Excellent communication and interpersonal skills, with the ability to build positive relationships with clients, families, and colleagues.
Compassion, patience, and a commitment to promoting the independence and dignity of people with disabilities.
Knowledge of health and safety, infection control, and safeguarding standards within a residential care setting.
A proactive and reliable attitude, with the ability to work both independently and collaboratively within a multidisciplinary team.
