About The Role: Bliss Nursing is looking for dedicated and caring Aged Care Workers / Disability Support Workers to join our professional team to provide person-centred support to older people and individuals living with disabilities, enabling them to live safely, comfortably and as independently as possible within their homes and communities across the following tasks: Deliver personal care assistance, including bathing, dressing, personal hygiene, grooming, and mobility support.
Provide practical assistance with everyday household tasks such as cleaning, laundry, and meal preparation.
Support clients to attend medical appointments, social outings, shopping, and community-based activities.




Encourage clients to build confidence, develop daily living skills, and achieve their individual goals.
Assist with medication prompts or administration in accordance with organisational policies and relevant training.
Foster positive relationships that promote dignity, respect, independence, and individual choice.
Accurately complete client documentation, progress notes, and incident reports in a timely manner.
Follow individual support plans while maintaining privacy, confidentiality, and cultural sensitivity.
Liaise effectively with families, allied health professionals, care coordinators, and other members of the multidisciplinary team.
Requirements: The ideal candidate will hold a Certificate III or IV in Individual Support (Ageing or Disability), or a relevant higher qualification in related disciplines with 1 year of experience in a similar role.
