RECOVERY SUPPORT WORKER FIXED-TERM FULL-TIME (6 MONTHS) HERSTON Your Next Opportunity!

Join Richmond Fellowship Queensland's Hospital to Home team in Herston in this full-time, 6-month fixed-term opportunity.
In this role, you'll provide evidence based, recovery oriented, non clinical psychosocial support to individuals transitioning from acute mental health wards back into the community, or those receiving support from community mental health services.
You'll play a vital role in helping people regain confidence, build independence and reconnect with the supports and opportunities that matter most to them.
What You'll Be Doing Supporting clients through hospital discharge planning and their transition back into the community.
Visiting clients in hospital and providing ongoing recovery focused support in their homes.
Working collaboratively with clients to achieve goals outlined in their Personal Recovery Plans.
Assisting with daily living tasks, skill development and capacity building.
Facilitating community connections and social inclusion opportunities.
Partnering with internal teams and external stakeholders to achieve the best possible outcomes for clients.
Managing your own client caseload, with regular supervision and robust team support.
In this role, you'll have the opportunity to make a genuine difference in people's lives by supporting individuals as they rebuild confidence,
strengthen independence and create meaningful lives within their communities.
You Will Have A Diploma in a human service related field and/or demonstrated experience working with people living with mental ill health.
The ability to work autonomously and professionally within RFQ's service philosophy and values.
Demonstrated ability to effectively manage time, your own caseload and KPIs, including outcome measures and safety planning.
Excellent communication and interpersonal skills, with the ability to effectively engage with clients, colleagues, stakeholders and service providers.
A strong sense of professional boundaries and integrity.
Willingness and ability to undertake vaccination against vaccine preventable diseases in accordance with the Occupational Assessment, Screening and Vaccination against Specified Infectious Diseases requirements.
