What you'll do

As an Assessment Partner in our Community Aged Care team, you'll work directly with older people in their homes to understand their care needs, goals and changing circumstances. Through comprehensive assessments and reviews, you'll help shape safe, practical and person-centred support plans that enable customers to remain independent and well at home.

Working autonomously across the community while staying connected to a supportive multidisciplinary team, you'll apply your clinical judgement to identify risks, respond to complex or escalated care needs and guide effective care planning. You'll also build trusted relationships with customers, families, carers and health professionals to support coordinated and positive outcomes.

You'll be trusted to make a valuable impact by:

- Conducting in-home assessments and reviews in line with customer goals, support plans, legislation and quality standards.
- Applying contemporary clinical knowledge and sound judgement to identify risks and guide care planning for complex or changing needs.
- Supporting customers to establish goals that promote independence, wellness and reablement.
- Providing clinical advice and guidance to care management teams within your professional scope of practice.
- Collaborating with families, service providers and health professionals to support coordinated care, advocacy and safe transitions between services.

What you'll bring

To thrive in this role, you will have:

- A tertiary qualification in Nursing,



Allied Health or another related health discipline, with current AHPRA registration where applicable.
- A minimum of two years' experience working autonomously in community aged care, home care or a comparable complex care environment.
- Strong assessment, care planning and clinical decision-making skills.
- The confidence to manage a varied workload, coordinate community visits and respond effectively to changing priorities.
- Clear and empathetic communication skills, with the ability to build trusted relationships with customers, families and skilled stakeholders.

We would also love you to have:
- Experience working within a person-centred wellness and reablement model.
- Confidence participating in case conferences and supporting customers with escalated or complex care needs.
- Strong documentation, reporting and risk-management skills.
