We are currently seeking experienced Community Worker (Team Leader) to join our compassionate and dynamic team.

This role is ideal for individuals with experience in supporting people with mental health conditions, physical disabilities, and challenging behaviors. You will work closely with an Area Supervisor, who will provide training, guidance, and support to ensure the highest standards of care are maintained.

About the Role:
As a Community Worker (Team Leader), your key responsibilities will include:

- Deliver person-centred assistance and connect clients with relevant relief and community support services.
- Conduct client assessments and implement tailored action plans to address social and economic needs.
- Identify service gaps within the community and liaise with agencies to improve access to essential supports.
- Represent and support disadvantaged groups to strengthen inclusion and community participation.
- Facilitate skill-building sessions to enhance self-reliance and everyday living capabilities.
- Provide immediate support and appropriate referrals during crisis or high-risk situations.
- Organise and oversee outreach activities, including community-based support and relief programs.
- Support clients to attend appointments and participate in social and wellbeing activities.
- Maintain accurate case notes, reports and documentation in line with organisational and compliance requirements.




- Collaborate with multidisciplinary teams and external stakeholders to coordinate comprehensive client support.
- Monitor client progress and review support strategies to ensure outcomes are effectively achieved.

About You:
To thrive in this role, you will bring:

- A minimum of 2 year of experience in supervising or leading a team of disability support workers.
- Experience supporting individuals with a range of disabilities, including mental health, epilepsy, autism, and challenging behaviors.
- Previous experience working across multiple group homes or SIL houses.
- Proficiency in basic Excel for creating weekly reports.
- A Certificate III in Disability (or similar/higher qualification).
- Strong communication skills and the ability to build rapport with both participants and team members.
- Empathy for the challenges faced by individuals with disabilities and a commitment to Person-Centered Practices.
- Organizational skills, a proactive mindset, and the ability to manage multiple priorities.
- A NSW Driver's License (or equivalent) and access to your own car is desirable.
